Health information means information about your health, including your medical history, information about any disabilities that you have or have had, information about any health services or disability services that are being provided, or have been provided, to you, information provided by you in connection with your donation of any body part or any bodily substance of that individual or derived from the testing or examination of any body part, or any bodily substance of yours or information about you which is collected before or in the course of, and incidental to, the provision of any health service or disability service to you.
The personal information and health information we collect
The types of personal information we may collect about you include:

- Contact Data including billing address, delivery address, email address and telephone numbers.
- Financial Data including bank account and payment card details.).
- Background Verification Data including details requested as part of our onboarding process to comply with our due diligence obligations, anti-money laundering laws and related ongoing monitoring commitments.
- Transaction Data including details about payments from you to us and other details of services you have purchased from us.
- Technical and Usage Data including internet protocol (IP) address, your login data, your browser session and geo-location data, your location information (for example your GPS location), device and network information, statistics on page views and sessions, acquisition sources, search queries and/or browsing behaviour, information about your access and use of our website, including through the use of Internet cookies, your communications with our website, the type of browser you are using, the type of operating system you are using and the domain name of your Internet service provider.
- Marketing and Communications Data including your preferences in receiving marketing from us and our third parties and your communication preferences.
- Professional Data including where you are a worker of ours or applying for a role with us, your professional history such as your previous positions and professional experience.

How we collect health information
Unless authorised otherwise, we collect health information from you directly in the course of providing our Services to you, including during our consultations in person, online, via email or over the telephone. We may collect health information about you from third parties or from you about your family members for the purposes of preparing a family or genetic history.

- to assess whether to take you on as a new client, including to perform anti-money laundering, anti-terrorism, sanction screening, fraud and other background checks on you;
- to provide our Services to you, including to assess and respond to your medical needs;
- to enable you to access and use our associated applications;
- to contact and communicate with you about our Services and any enquiries you make via our Website;
- for internal record keeping, administrative, invoicing and billing purposes;
- for analytics, market research and business development, including to operate and improve our Services, Website and associated applications;
- to detect and/or prevent any illegal activity that may threaten us or our Services;
- for advertising and marketing, including to send you promotional information about our products and services and information that we consider may be of interest to you, noting we will comply with all laws that are relevant to marketing (including the Unsolicited Electronic Messages Act 2007 and Fair Trading Act 1986);

- third party service providers for the purpose of enabling them to provide their services, to us, including (without limitation) practice management software providers, IT service providers, data storage, web-hosting and server providers, debt collectors, couriers, maintenance or problem-solving providers, marketing or advertising providers, professional advisors and payment systems operators;
- our employees, contractors and/or related entities;
- our existing or potential agents or business partners;
- anyone to whom our business or assets (or any part of them) are, or may (in good faith) be, transferred;
- courts, tribunals and regulatory authorities, in the event you fail to pay for goods or services we have provided to you;

What is HealthOne?
HealthOne is a south island based secure electronic record that allows registered healthcare providers directly involved in your healthcare, to quickly access information such as your test results, allergies, medications, GP summaries and hospital information.
HealthOne strictly adheres to the principles of the Privacy Act, 2020 as well as the Rules set out in the Health Information Privacy Code 2020. Access is only possible via an approved highly secure healthcare information network which is regularly audited and tested. Privacy auditing is used to check that only those directly involved in your care are accessing your information.

Overseas disclosure: Where we disclose your personal information and/or health information to third parties listed above, these third parties may store, transfer or access personal information outside of New Zealand which may not have an equivalent level of data protection laws as those in New Zealand. Before disclosing any personal information to an overseas recipient, we will comply with Information Privacy Principle 12 and Rule 12 of the Health information Privacy Code 2020 only disclose the information if:
- you have authorised the disclosure after we expressly informed you that the overseas recipient may not be required to protect the personal information and/or health information in a way that, overall, provides comparable safeguards to those in the Privacy Laws;
- we believe the overseas recipient is subject to the Privacy Laws;
- we believe that the overseas recipient is subject to privacy laws that, overall, provide comparable safeguards to those in the Privacy Laws;
- we believe that the overseas recipient is a participant in a prescribed binding scheme;
- we believe that the overseas recipient is subject to privacy laws in a prescribed country;
- we otherwise believe that the overseas recipient is required to protect your personal information in a way that, overall, provides comparable safeguards to those in the Privacy Laws (for example pursuant to a data transfer agreement entered into between us and the overseas recipient); or
- disclosure is in accordance with an authorisation granted under section 30 of the Privacy Act 2020.
Your rights and controlling your personal information and health information

If you provide personal information and/or health information to us, you understand we will collect, hold, use and disclose your personal information and health information in accordance with this Privacy Policy. You do not have to provide personal information to us, however, if you do not, it may affect our ability to provide our Website and Services to you and your use of our Website and Services.
Information from third parties: If we receive personal information and/or health information about you from a third party, we will protect it as set out in this Privacy Policy. If you are a third party providing personal information and/or health information about somebody else, you represent and warrant that you have such person’s consent to provide the personal information to us.
